How to complete your onboarding form
The onboarding form is the one thing the mediator needs from you before anything else can happen. It confirms who you are, where the invoice goes, and what the case is about. Your invoice isn't raised until it's submitted.
Before you begin
Open the link in your onboarding email. It goes straight to your form — there's no password and nothing to create. The page is headed with your side and the case name, so check it says the right one before you start. If you're the plaintiff's counsel it reads Plaintiff Onboarding Form; if there are several parties on your side it names yours specifically.
1Your details and who pays
Name on Invoice is the person responsible for payment, with the firm name. Billing Address is the full address including city, state and zip. Both are required.
Below them you can add other addresses that should receive the invoice — a billing clerk, an accounts inbox.
2The case information
A set of questions about the matter. Which ones you see depends on the mediator; not every mediator asks for all of them. Typically:
- Trial Date
- Referring Judge
- Case Type
- Causes of Action
- Remaining Discovery
- Remaining Law and Motion
- Your last demand or last offer
Answer what applies. What you enter here appears on the mediator's case page, so it's the mediator's briefing rather than a form for its own sake.
3Your documents
Upload boxes for the documents your side is expected to file. Plaintiff's counsel is usually asked for the Operative Complaint and the Service List; both sides for the Brief and Additional Documents.
A red asterisk means required. Anything without one is optional.
PDF, JPEG, JPG, PNG, XLSX, XLS, CSV, DOC and DOCX are supported, up to 30 MB per file and five files per box.

4Anything else
Additional Information is a free-text box at the bottom for anything the questions didn't cover.
5Submit
Submit the form. You'll see a confirmation page.
Your onboarding then shows as complete on the mediator's side, the reminders about it stop, and your invoice is raised and emailed to you.
If you lose the link
Ask the mediator. They can copy your onboarding link straight off the case and send it to you, or trigger the onboarding email again. It's specific to you, so don't use a colleague's.
